Even though we are a “paperless” office with electronic medical records, paper forms are unfortunately still necessary.
You can complete the necessary forms in advance before your appointment, or you can fill them out in our office. PLEASE arrive 15 minutes before your first appointment if you are filling out the forms in our office.
Patients who have seen us before with an older child and now have a new baby will need to complete the new patient forms.
